Check the following items and make sure they are completed before your personal assistant (PA) leaves.

Give the PA written notice of his or her discharge, or acknowledgement of the resignation. |

Give the PA the final paycheck, or submit the final timesheet to the agency who is paying the PA. |
 |
Obtain a forwarding address for sending tax information and get a phone number if future contact is necessary. |

Make sure that your former PA signs a form confirming that s/he received the last paycheck. |
 |
Retrieve keys to car, house, or other properties. |
 |
Retrieve any possessions the PA may have borrowed. |
 |
Return any possessions you may have borrowed. |
 |
Take stock of your supply of medications, check for discrepancies. |
 |
Remind the PA of his or her confidentiality obligations concerning your private life. |
 |
Make sure that the PA returns your credit card, and any financial information s/he may have. You should return anything you have belonging to the PA. |
 |
Ask, and record the PA's primary reason for leaving. |
 |
Ask, and record whether the training was adequate, and how it could be improved. |
 |
Ask, and record whether the PA was content with the working environment and how it could be improved. |
 |
Ask, and record suggestions for improving employee morale. |
 |
Ask, and record what they thought the best part of the job was. |
 |
Ask, and record what they thought the worst part of the job was. |
