Job applications

Applicants should be asked to fill out a job application form and to provide you with references.

The job application form serves a several purposes:

  • It gives you more information about the skills of the applicant.
  • It gives you another screening tool. It helps you to decide if the applicant is suitable and worth another interview.
  • It helps you to judge the applicant's literacy if it is important to you that they read and write clearly.
  • It shows you how well they follow directions.
  • It helps the applicant to view this as a serious job.
  • It provides a record for your files.

There are a two ways to proceed.

  1. If you are impressed by the telephone-screening interview, you may ask to send them an application in the mail. Be sure to ask them for their addresses. Send the job application and the job description with a short cover letter that states the date you need it to be returned to you.
  2. You can give them the job application at the first in-person interview. If you choose this option, allow enough time in the interview to complete it.

Sample job application form

The job application form is designed so you can personalize the application to fit your situation.

Now is a good time to develop your job application form so you will be ready for the interviewing process. You can view a sample job application to help you get started. You can use as much or as little of this sample as you like in creating your own job application form.

WarningIf you live alone, for your safety, you may wish to have the application returned to another address. You could use the address of a family member or friend. Sometimes an organization that serves people with disabilities will let you use its address. Example: Return to Joe Smith c/o Karen Jones, 222 Walden Street, Newton, OK; or Joe Smith c/o Kern County Office of Aging, 3120 Orange Street, Baldwin, PA.

Sample cover letter

Whether you mail or hand the application to candidates in person, a brief cover letter can include information about the date the application must be returned to you, and where the application should be sent. Below is a sample letter.
