Description:
This waiver serves persons 60+ and having a nursing facility level of care. This waiver is not open to new enrollees. Consumers must first be on the Ohio Home Care Waiver and be "transitioned" to the Transitions Carve-Out Waiver program due to turning 60 years old. Services in this waiver include: personal care, nursing and adult day health.
