Health Management Associates (2005). Department of Public Health 2005 Report: Its Effectiveness as an Integrated Health Care Delivery System and Provider of a Continuum of Long Term Care Services. Lansing, MI, Author.
Abstract
This report is the findings of the evaluation of the effectiveness of the continuum between acute and long-term care services provided by the San Francisco Department of Public Health (the Department). The report examines the mission, structure, leadership and operation of the Department as a “seamless” continuum of care and makes specific recommendations to assure greatest possible integration. It also presents the current state and the appropriateness of long-term care services that are and should be available for the population that the Department has determined that it serves and proposes actions to make those services more effective.
The report finds that the City needs to substantially improve the integration of its hospitals and community-based services. San Francisco can better meet the needs of its citizens and gain significant financial benefits by not increasing the size of the Laguna Honda Hospital rebuild project and instead providing a mix of long term, skilled nursing, in-home and community-based services.
